How does the ClubZap Medical Disclosure work?
As of December 2024 we have introduced a new Medical Disclosure section on Memebrship forms which provides an option for member to toggle on or off whether they would like to disclose if they have medical conditions that need to be brought to the attention of the club and/or coaches.
When the toggle option is enabled (green) the person is not asked for the specifics of their medical disclosure but instead it will set a flag against their profile which will indicate to the club/coach that they need to follow up with the member for more detailed information. The member is advised to provide the coach with more information on this also.
How can I view a list of members who have provided medical disclosure on their memebrship form?
Login to clubzap.com/signin
Go to Membership -> Members
Click on the Filters button
Select Medical Disclosure
Select Disclosed to view a list of your members who have the Medical Disclosure option selected on their memebrship form.
Note: The Medical Disclosure Filter can be used in conjunction with any of the other filters such as gender and DOB. e.g: Filter on Medical Disclosure for Boys Born in 2010
How can I export a list of members who have provided medical disclosure on their memebrship form?
Login to clubzap.com/signin
Go to Membership -> Members
Click on the Export button
Under the Select Attributes list toggle on Medical Disclosure. You can further refine your export on other variables such as Date Paid etc before completing the Export to a .csv file.
How can a coach see who they need to follow up with regarding Medical Disclosure?
Provided your club Registrar has Squad reports setup as detailed here, then team coaches will receive an automated email on Friday mornings summarising
Coaches and Players who have and have not paid membership
Coaches and Players who are and are not registered with the governing body
Members in their squad who have opted in to a Medical Disclosure and need following up with.
